Clinical Diagnostic Reference Levels in Interventional Neuroradiology in Qatar

Purpose: The objectives of this study were to: 1) evaluate patient radiation exposure in neuroradiology interventional (INR) procedures and 2) establish Diagnostic Reference Levels (DRL)s for stroke (S), brain aneurysms (AN) and brain arteriovenous malformation (AVM) clinical indications.

Methods: Adult patient data undergoing cerebral endovascular treatment for S, AN and AVM were retrospectively collected using specially designed collection forms. The methodology described in the International Commission on Radiological Protection (ICRP) Report 135 was followed to establish clinical DRLs in terms of Kerma-Area Product (KAP), frontal (CDf) and lateral Cumulative Dose (CDl), fluoroscopy time (T), total number of images (Ni) and total number of runs (Nr). Collection forms were prepared in excel format in order to assist data analysis. The forms aimed to include patient demographic information (gender, age, weight, height, etc) X-ray modality technical parameters (fluoroscopy time (T), number of images (N), number of runs (r) and operator information (contact details, years of experience). Only adult patients (16≥ years of age) would be included in the study.

Results: This study included data from 209 adult patients. National DRLs in terms of KAP, CDf, CDl, T, Ni and Nr for S, AN and AVM were 78 Gy cm2, 380 mGy, 118 mGy, 12 min, 440, 157 for stroke, 85 Gycm2, 610 mGy, 96 mGy, 20 min, 720, 20 for AN and 180 Gy cm2, 1144 mGy, 540 mGy, 36 min, 1380 and 30, respectively. Comparison was restricted as most recent literature refers to the technique and not the clinical indication.

Conclusion: This is the first study reporting national DRLs based on clinical indication for INR procedures. The established national clinical DRL values will further facilitate patient dose optimization and quality improvement process within Hamad Medical Corporation and the state of Qatar.
