The ability of magnetic resonance imaging (MRI) to create distinct images of the body’s soft tissues and the tumors that arise amid them is helping Brachytherapy Radiation Oncologists to precisely shape doses to cervical and prostate tumors, while at the same time avoiding exposure to critical healthy organs and tissues.
Conforming brachytherapy doses closely to the tumor’s shape and position is important in order to apply the highest possible dose to the tumor while limiting the dose to critical structures, such as the bladder rectum, and urethra thereby decreasing the likelihood of treatment side effects.

To achieve the above, comprehensive quality assurance and safety considerations need to be put in place prior to clinical use of MRI is Radiation Oncology imaging and treatment planning.

There are three approaches to incorporating MRI into the brachytherapy workflow. They are: MR-informed MR-guided, and MR–based brachytherapy. Workflows for MR based HDR brachytherapy will be discussed, however, many of these processes apply to MR-guided techniques as well. Safety consideration will be reviewed and their implications to applicator, and workflow selection.

Implementation of MR-based HDR gynecologic and prostate brachytherapy will be discussed, including development of an imaging protocol, applicator commissioning and reconstruction, simulation techniques, contouring, treatment planning and dose adaptation.
