The purpose of this session is to complete the picture of breast cancer diagnosis and treatment in a joint education session for diagnostic and therapy medical physicists.
Diagnostic: In the “diagnostic” portion of this session, the use of imaging in a breast clinic beyond screening mammography will be presented. The session will begin with an overview of the diagnostic workup of commonly recalled imaging findings seen on screening mammography including the purpose and terminology of diagnostic mammographic views and the performance benchmarks of diagnostic mammography. Furthermore, modalities other than mammography will be discussed, in particular, the role of MRI and whole-breast 3D ultrasound. A co-registration technique of breast tomosynthesis and whole breast ultrasound will be presented, and discussion of the benefits of cross-modality breast registration. Last, the types of imaging utilized during biopsy procedures for margin assessment of biopsy specimen will be explained.
Therapy: In the “therapy” portion of this session, we will focus on APBI and SBRT. For breast radiotherapy, whole breast radiotherapy (WBRT) has been the standard of care. Nevertheless, accelerated partial breast irradiation (APBI) is also commonly recommended for early-stage breast cancer with advantages including shorter treatment course and smaller target volume than WBRT. Outcome of trials have shown equivalent tumor recurrence rate and overall survival between two types of treatments. With the excellent outcome of APBI, interest in stereotactic body radiation therapy (SBRT) for partial breast has increased recently. The treatment part of this education session proposes to review the current practices of breast SBRT and APBI using various treatment technique. The treatment process starting from diagnostic imaging to treatment delivery will be presented in this session. Main focus will be in treatment planning and delivery technique with Linac, GammaPod and Proton.
