Radiomic Analysis of Tumor Heterogeneity in HCC Post-Chemotherapy Using MRI

Purpose: To assess the interval changes in the heterogeneity of diffusion-weighted apparent diffusion coefficient (ADC) maps and T1-weighted MRI of gadolinium in hepatocellular carcinoma (HCC) with and without chemotherapy-radiotherapy (CRT) response based radiomic signature.

Methods: This retrospective observational cohort study included 36 patients (30 men and 6 women, age 54.3 ± 7.9 years) with stage III or IV HCC. Radiomic features of the primary tumor (n=36) were measured by ADC maps and T1w post-gadolinium sequences, performed before treatment and 8 weeks after CRT. We extracted radiomic features by using the PyRadiomics platform implanted with 3D Slicer software. These features were classified into several categories including shape, first order, gray scale co-occurrence matrix (GLCM), gray scale dependence matrix (GLDM), gray scale run length matrix (GLRLM), gray scale size zone matrix (GLSZM), and neighboring gray scale difference matrix (NGTDM). Treatment response was recorded for 1 year at the time of initial diagnosis. Wilcoxon signed rank test was used to compare the interval changes in parameters after treatment response and failure stratification (p<0.05 statistical significance).

Results: 36/36 primary tumors responded to CRT within 1 year. Responding HCC showed significant interval changes in ADC histogram parameters (maximum, kurtosis, entropy, energy, and run variance) and gad GLCM (entropy, run entropy, and contrast) after T1w at 8 weeks after CRT (P<0.05).

Conclusion: The ADC maps and T1w post gad MRI radiomic signature show the evolution of heterogeneity parameters in successfully treated HCC at 8 weeks after CRT.
