Study On the Method and Accuracy of Volume Delineation and Motion Amplitude Measurement Based On Four-Dimensional CT Images Under Different Breathing Motion Modes

Purpose: To study the method and accuracy of volume delineation and motion amplitude measurement based on four-dimensional CT images in different breathing modes.

Methods: CIRS respiratory motion phantom was used to drive the small ball with gold (target section) labelled antigen detection method implanted in the middle of the phantom to move in four modes: standard sine, end expiratory stagnation, expiratory time longer than inspiratory time, and amplitude instability. 4DCT scanning was carried out for ten uniform phrases, the target volume was delineated and the results were analyzed. At the same time, the measurement accuracy of the gold standard method and the centroid method was analyzed.

Results: The target volume delineated on CT images of 40%, 50% and 60% phases in the regular breathing mode is relatively stable and closer to the static target volume than the delineation results on other phase images. The average error of judging motion amplitude by gold labelled antigen detection method was 10%; The mean error of motion amplitude judged by gross tumor volume (GTV) centroid was 36.2%.

Conclusion: Based on 4DCT scanning, 40%-60% phase (3 phrases) is recommended for target volume delineation and treatment during the respiratory gating therapy.Stable respiratory motion is more favorable for image reconstruction of target volume delineation and amplitude judgment; At the same time, the accuracy of obtaining tumor motion amplitude by gold labelled antigen detection method is higher than that by centroid method.
